Apprentice Roadmap to Licensure

Step 1 - Apply for a Certified Apprenticeship - Embalmer Only

APPRENTICESHIP DESIGNATION:

EMBALMER ONLY

12-month term

 

An individual seeking to apply for a Certified Apprenticeship as an Embalmer Only, must complete and submit the following items to the Board: 

  1. Request an Official College Transcript(s)
    (The Board must receive the transcript(s) directly from the college and/or university)
     
  2. Complete a BCI & FBI Criminal Background Check from an authorized location
    (Click Here for Locations)
    (The Board must receive the BCI & FBI Criminal Background results directly from the Ohio Attorney General, not from a licensee nor any other entity)
     
  3. Create an Ohio eLicense account
    (Click Here for Instructions)
     
  4. Once logged in to your account, click "APPLY FOR A NEW LICENSE"
    Under Select a License, a drop down menu will list all of the Boards' available license applications. 
    At the top of the drop down menu choose
     
    "Certified Embalmer Apprenticeship".
     
  5. Complete the application questions and upload the requested documents, including a copy of the Master's Certificate.
     
  6. Pay the online application fee

  7. NOTE:  STEPS 4, 5, and 6 MUST BE COMPLETED TWICE IF APPLYING FOR BOTH AN EMBALMER APPRENTICESHIP AND A FUNERAL DIRECTOR APPRENTICESHIP

Step 2 - Print Apprentice Task List and Master's Checklist of Required Tasks - Embalmer Only

Once your Embalmer Only Apprenticeship has been certified by the Board Certification and Licensure Examiner, you will be ready for Step 2.

This is your primary checklist which you will be working from during your Apprenticeship.

You will not be submitting this form to the Board.

Next, PRINT the Master's Checklist of Required Tasks and give it to your Master.

All necessary tasks for your Apprenticeship are contained in this checklist which is to be used by your Master.

You and your Master will be working from these lists to ensure you are learning the required tasks for your Apprenticeship.

You will not be submitting this form to the Board.

Step 3 - File Quarterly Reports - Embalmer Only

Both the Apprentice and Master are responsible for completing Quarterly Reports.

All required reports will be submitted by the Apprentice through your eLicense account.

This document will provide you with valuable information on when to submit and how to label your electronic files for uploading to eLicense.

You will not be submitting this form to the Board.

PRINT the Apprentice Quarterly Case Report - Embalming Case using the link below.

This form is to be completed by the Apprentice. You will fill out 1 form per Decedent.

You will be submitting this form to the Board.

APPRENTICE QUARTERLY CASE REPORT - Embalming Case OBEFD008

 

PRINT the Master's Quarterly Evaluation Report from the link below and give the printed form to your Master.

Your Master will fill out 1 Evaluation Report per quarter. 

This report is intended to provide both the Master and the Apprentice with a method of tracking the progress of the Apprentice. Degrees of knowledge and proficiency are recorded in this report, along with requests for additional training, and requests for contact with an Inspector.

Both you and your Master must sign the Master's Quarterly Evaluation Report.

You will be submitting this form to the Board.

Terminating Your Apprenticeship - Embalmer Only

At the end of your 12-month Embalmer Only Apprenticeship, you will need to follow the process for Terminating your Apprenticeship.

You will then be able to apply for your Initial Embalmers License.

 

First, you will need to DOWNLOAD and PRINT the following forms.

After completing the forms, you will need to SCAN the completed pages and SAVE the forms to your hard drive as .pdf files.

When you have all of your documents ready, you will log into your eLicense account.

On your Dashboard under OPTIONS select TERMINATION OF APPRENTICESHIP. Use SUBMIT ADDITIONAL DOCUMENTATION to upload your files.

You will also need to upload a recent photo of yourself using the same procedure for the forms.
