The following items are required in order to apply for a change in ownership:

1. A letter stating an intent to close the facility, the name and license number of the actually in charge and ultimately responsible, the date facility closure will become effective, the name and license number of the existing facility that is closing. This letter must be uploaded in eLicense;
2. Return the wall display and wallet card to the Board office via mail; and
3. Submit a clearly enumerated account of all preneed funeral contracts, life insurance policies or annuities which are payable to the provider of funeral or burial goods or services; and accounts at banks or savings banks insured by the federal deposit insurance corporation.
4. Send written notice to the purchaser of every preneed funeral contract to which the funeral business is a party via first class United States Mail. Such notice must be addressed to the purchaser's last known address and shall explain that the funeral business is being closed and the name of any funeral business that has been designated to assume the obligations of the preneed contract. 

An individual seeking to close a facility is required to complete a service request online through eLicense. Closing a facility is important to the Board requires the actually in charge and ultimately responsible to carefully follow the provisions set forth in the Revised Code.

The actually in charge and ultimately responsible is required to log into eLicense, access the service request feature, and request to inactivate the current license. The current actually in charge and ultimately responsible must upload the required letter notifying the Board of the closure, upload the notice sent to the purchaser of every preneed funeral contract, and return the wall display and wallet card.

The Revised Code requires the notices to be sent within 30 days of closing of a funeral home.
